Curtis Kodama is a board certified Family Physician who completed his undergraduate at UC Irvine. He also earned a degree in Osteopathic Medicine at Western University Health Sciences in Pomona, CA. He completed his residency at USC/ California Medical Center in Los Angeles. He grew up in San Fernando Valley and is married with 2 sons. His hobbies include competition swimming, softball, basketball, camping and fishing. He is Member of AAFP, Rotary Club of Moorpark, and Boys and Girls Club of Moorpark. He is also the team physician for Moorpark High School.

Crystal Pollard is a board certified Physician Assistant who graduated from Stanford University School of Medicine. She also earned a Master's degree in Medical Science, a Bachelor's degree in Healthcare Administration, and two Associate's degrees in Applied Science. She is also a Moorpark Rotarian and participates in various local Boys and Girls Club fundraising events. She graduated from Moorpark High School as a member of the Health and Science Academy in 2005. Her hobbies include traveling, painting, camping, and her dog, Boo.

Stanley Su is a board certified Family Physician, who completed his BS from the University of Minnesota, his MD from University of the East, Taiwan and completed his internship and residency in Family Practice at Wright State University in Ohio and St. John Medical Center in Michigan. He has practiced family medicine in Ventura County for the past 30 years. He is married with 2 daughters, living in Thousand Oaks. His hobbies include tennis, Hebrew study and bible teaching. He is a member of AAFP and CMDA.

Monica Su is a board certified Family Physician, who completed her BS at UC Santa Barbara. She recieved her MD degree from Ross University and completed her residency in Family Practice at Hofstra North Shore Long Island Jewish Medical Center in New York. Then she joined her father in private practice in Newbury Park. She was born and raised in Ventura County and is married. Her hobbies include running, traveling and dogs. Special interests include sports medicine and women’s health. She is a Member of the AAFP and ACSM.
